Research suggests that ergothioneine holds meaningful promise as a neuroprotective compound, with a broadly supportive body of evidence spanning one small randomized controlled trial, multiple systematic and narrative reviews, and a range of preclinical animal and cell culture studies. Studies indicate that ergothioneine may protect the brain through several converging mechanisms, including antioxidant and anti-inflammatory activity, support for mitochondrial function, promotion of neurogenesis, and modulation of neurotrophic signaling, with the compound shown to cross the blood-brain barrier via a specific transporter and with lower circulating levels consistently observed in people with cognitive impairment, Parkinson's disease, and frailty compared to healthy individuals. The sole human clinical trial, while encouraging in showing stabilized neuronal damage markers and improved verbal memory in older adults with mild cognitive impairment, involved only 19 participants over one year, and the authors themselves caution that its findings are preliminary. Overall, while the preclinical evidence is mechanistically rich and directionally consistent, the field lacks large, well-powered human trials, meaning the existing research, though promising, is not yet sufficient to draw firm conclusions about ergothioneine's effectiveness as a neuroprotective intervention in humans.
