Research suggests that Vitex agnus-castus (chasteberry) shows meaningful promise for breast-related concerns, particularly cyclic mastalgia — the recurring breast pain many women experience before their periods — with two meta-analyses pooling data from dozens of clinical trials reporting consistent reductions in both breast pain intensity and elevated prolactin levels compared to placebo or active comparators. Several randomized controlled trials and a large retrospective study of 1,700 women further support these findings, with one well-designed RCT reporting that roughly twice as many women taking the extract experienced clinically meaningful symptom relief compared to placebo, and the retrospective study documenting breast tenderness falling from approximately 40% to under 1% after three months of use. However, the evidence carries important caveats: reviewers consistently note that many included studies have unclear or high risk of bias and lack methodological rigor, a 2006 review raises the concern that chasteberry may actually reduce milk supply in breastfeeding women rather than support it, and a 2023 pharmacovigilance analysis identified reproductive and breast-related adverse reactions among reported side effects, underscoring that the safety profile of these supplements is not yet fully characterized. Taken together, the body of evidence is encouraging but not definitive, and readers should be aware that product quality can vary considerably given that dietary supplements are not subject to the same pre-market approval standards as medications.
