Research suggests that Tribulus terrestris may support sexual function and libido in both men and women, with the bulk of available evidence coming from small randomized controlled trials and narrative reviews that generally point in a positive direction. Studies indicate improvements in desire, arousal, and satisfaction in women with low sexual desire, including those who are premenopausal or postmenopausal, and some trials in men with mild to moderate erectile dysfunction have reported meaningful gains on validated sexual function questionnaires. However, the evidence is not uniformly positive — at least one placebo-controlled trial in men found no statistically significant benefit over placebo at the dose and duration tested — and a key 2016 review challenged the widely held assumption that any effects are driven by raising testosterone, instead pointing to nitric oxide pathways as a more plausible mechanism that has yet to be well studied in humans. Across the literature, sample sizes tend to be small, study durations short, and several trials examined Tribulus terrestris in combination with other herbal ingredients rather than in isolation, all of which makes it difficult to draw firm conclusions about how reliably it works or precisely how it acts.
