Tongkat Ali (Eurycoma Longifolia) for Testosterone Support

Preliminary evidence 12 studies

Research suggests that Eurycoma longifolia (Tongkat Ali) shows promising evidence for supporting testosterone levels and related androgenic activity in men, with a 2022 systematic review and meta-analysis of randomized clinical trials finding a statistically significant increase in total testosterone compared to controls, an effect that held up even in men with clinically low testosterone. Studies indicate that much of the supporting evidence comes from animal studies and small human pilot trials, with proposed mechanisms including inhibition of aromatase (the enzyme that converts testosterone to estrogen) and modulation of the hypothalamic-pituitary-gonadal axis, as identified in cellular and preclinical research. A pilot study in older adults found increases in both total and free testosterone alongside improvements in grip strength in men and women, though the overall body of human clinical evidence remains limited in scale and rigor, and the systematic review's authors specifically noted that larger, more rigorous trials are needed before clinical recommendations can be made. One case report raised a potential cardiovascular safety concern — a case of atrial flutter temporally associated with supplementation — which, while a single isolated report, serves as a reminder that the safety profile of the herb has not been comprehensively established in humans.
