The available evidence includes a 2022 systematic review and meta-analysis of randomized controlled trials that found a statistically significant increase in testosterone in men, including those with hypogonadism, along with multiple animal studies identifying quassinoids and the compound eurycomanone as likely active contributors to these reproductive effects. Studies indicate that at concentrations relevant to typical supplementation, the extract does not appear to cause harm to sperm function, and a two-generation rat toxicity study found improvements in fertility outcomes with no observed adverse effects at tested doses, though these animal findings cannot be directly extended to humans.
