Saw Palmetto for Testosterone Support

Moderate evidence 12 studies

Research suggests that saw palmetto's most studied mechanism relevant to testosterone involves inhibiting 5-alpha-reductase, the enzyme that converts testosterone into the more potent androgen dihydrotestosterone (DHT), with in vitro studies, animal models, and at least one small randomized controlled trial showing modest reductions in prostate tissue DHT levels. Studies indicate, however, that this effect does not translate into meaningful changes in circulating testosterone levels, as multiple randomized controlled trials testing saw palmetto in combination supplement formulas found no increase in total testosterone, and one trial actually observed unwanted rises in DHT and estradiol alongside the supplementation. The body of evidence here is mixed in direction and limited in quality, drawing heavily on preclinical animal and cell studies, a small number of human trials with confounded designs (saw palmetto was rarely tested in isolation), and one veterinary study that found no effect at all in dogs with prostate enlargement. Overall, the current research does not support saw palmetto as a reliable means of raising testosterone, while its role in modestly influencing DHT metabolism in prostate tissue remains an area of ongoing, if inconsistent, scientific interest.
