Human RCTs found that both oral rosemary supplementation and aromatherapy inhalation were associated with improvements in prospective and retrospective memory, reduced anxiety, and greater alertness, while animal studies showed spatial memory improvements alongside reductions in amyloid-beta, neuroinflammation, and increased neurotrophic factors in the brain. However, the evidence is not uniformly consistent — one RCT examining aromatherapy in hemodialysis patients found no statistically significant between-group differences despite within-group improvements, and a broader review of herbal compounds for Alzheimer's disease noted that clinical trial results have been inconsistent overall. Studies to date are generally small, vary considerably in methodology, population, and form of rosemary used, and researchers across multiple reviews emphasize that larger and longer-duration human trials are needed before firm conclusions can be drawn.
