Research suggests that ginseng and its bioactive compounds, particularly ginsenosides and polysaccharides, may exert a range of immunomodulatory effects, with several animal studies reporting enhanced natural killer cell activity, improved macrophage function, restored immune organ size, and upregulated cytokine production under conditions of chemotherapy- or radiation-induced immune suppression. The available evidence comes primarily from laboratory and animal studies, with only a small number of randomized controlled trials in humans, which have returned mixed results — one crossover trial in healthy men found only limited immune changes following ginseng supplementation, while another review of herbal supplements in athletes highlighted persistent contradictions across studies. Studies also indicate that different ginseng preparations, such as polysaccharide-rich extracts versus ginsenoside-rich extracts, appear to influence immune pathways differently, complicating direct comparisons across research. Overall, while the body of preclinical evidence is suggestive of immune-supporting potential, the human clinical evidence remains limited and inconsistent, and significant gaps in understanding the precise mechanisms and practical benefits for human immune health persist.
