Research suggests that horse chestnut seed extract, particularly its active compound escin, shows meaningful promise for reducing edema associated with chronic venous insufficiency, with one well-designed 1996 randomized controlled trial finding it comparable in effectiveness to compression stockings over 12 weeks, and several supporting reviews and smaller studies reporting reductions in leg swelling and related symptoms. Studies indicate that the anti-edema effects may stem from escin's ability to reduce capillary fluid leakage and, in animal models, potentially promote lymphatic vessel development, with a 2025 laboratory and animal study also reporting substantial anti-inflammatory effects from a related species. However, the evidence is notably mixed when it comes to post-thrombotic syndrome specifically, where multiple Cochrane meta-analyses pooling data from three small trials found no statistically significant benefit over placebo or compression stockings, rating the overall quality of that evidence as low to very low. Several of the supporting studies also involve combination products, small sample sizes, or lack blinding, which limits the conclusions that can be drawn, and the research as a whole suggests that while horse chestnut preparations may be a reasonable option for certain venous conditions, the strength and consistency of evidence varies considerably depending on the specific application being studied.
