Ginkgo Biloba for Cognitive Decline Support

Research suggests that Ginkgo biloba extract, particularly the standardized form known as EGb 761, shows meaningful promise for supporting cognitive function in older adults with mild cognitive impairment and dementia, with multiple reviews and observational studies reporting improvements in cognition, neuropsychiatric symptoms, and daily functioning. Studies indicate that proposed mechanisms include antioxidant activity, mitochondrial protection, anti-amyloid effects, and modulation of neuroprotective signaling pathways, lending some biological plausibility to the clinical observations. However, the evidence base is not uniformly positive — at least one well-designed randomized controlled trial conducted in nursing home residents found no significant benefit over placebo for dementia or age-associated memory impairment, and another RCT found no effect on chemotherapy-related cognitive changes, illustrating that results vary considerably across populations, settings, and study designs. The overall body of evidence leans supportive, particularly at higher daily doses and in community-dwelling adults with early cognitive changes, but the presence of null findings from controlled trials, the relatively small size of several positive studies, and the limitations inherent in retrospective designs mean these findings should be interpreted with caution pending larger, more rigorously controlled prospective research.
