Research suggests that evening primrose oil has not demonstrated meaningful effectiveness for cyclical breast pain (mastalgia) or fibrocystic breast conditions, with the strongest evidence coming from a 2021 meta-analysis pooling data from 13 randomized controlled trials and over 1,700 women, which found no significant benefit compared to placebo or other treatments such as topical NSAIDs. Studies indicate that earlier clinical interest in EPO for breast pain, reflected in its historical use among UK breast specialists as a lower-risk first-line option, has not been substantiated by more rigorous subsequent research, and a 2017 comparative trial found that while flaxseed showed a statistically significant reduction in pain duration, EPO's trend toward improvement did not reach significance and could not be distinguished from the other interventions. A 2000 review of nutritional approaches to fibrocystic breast conditions similarly concluded that no dietary or supplement-based intervention, including EPO, is supported by sufficiently robust trial data, noting that the condition's natural variability and a substantial placebo effect make benefit difficult to measure. On the positive side, across the available studies EPO appears generally well tolerated, with no notable increase in adverse effects reported, though researchers consistently emphasize that the evidence base remains methodologically limited and that better-designed trials are needed before any firm conclusions can be drawn.
