Studies indicate that CoQ10 is consistently described in the literature as an essential cofactor in mitochondrial energy production, and several trials focusing on populations with chronic fatigue conditions, including ME/CFS and post-viral fatigue syndrome, have reported meaningful reductions in fatigue scores, particularly when CoQ10 is combined with NADH or selenium. However, the overall body of evidence carries notable limitations, including small sample sizes, high risk of bias, and heterogeneous study designs, and at least one RCT in chronic kidney disease patients found no significant improvement in aerobic capacity despite observable metabolic changes. Taken together, the research is promising but not yet definitive, and stronger, more rigorously designed trials are needed before firm conclusions can be drawn about CoQ10's role in energy and fatigue outcomes across broader populations.
