Research suggests that citicoline has been extensively studied as a neuroprotective agent in acute ischemic stroke, with a body of evidence spanning preclinical work, older supportive clinical reviews, randomized controlled trials, and several meta-analyses — though the overall picture from higher-quality modern evidence is mixed. Studies indicate that while earlier reviews and pooled analyses involving large numbers of patients reported modest potential benefits, particularly in older patients or those who did not receive clot-dissolving therapy, more rigorous recent evaluations tell a different story: a 2020 Cochrane meta-analysis of over 4,000 patients found little to no meaningful difference between citicoline and placebo on outcomes including survival, independence, and neurological recovery, and a 2022 randomized controlled trial similarly found no added benefit when citicoline was given alongside standard recanalization therapy. A 2023 meta-analysis further noted that a related compound, choline alphoscerate, outperformed citicoline on several stroke outcome measures, and a 2024 network meta-analysis ranked other neuroprotective agents above citicoline in overall effectiveness and safety balance. Citicoline does appear to have a favorable safety profile across the studies reviewed, but the limitations of existing research — including high risk of bias in many included trials, low overall evidence quality, and inconsistent findings across populations and study designs — mean that firm conclusions about its clinical utility in stroke cannot yet be drawn.
