Research suggests that butterbur, particularly extracts derived from Petasites hybridus and related species, shows some promise as a supportive option for allergic conditions including asthma and allergic rhinitis, though the evidence is mixed and often limited by study design. Studies indicate that active compounds in butterbur, known as petasins, can inhibit leukotriene and histamine production through mechanisms relevant to allergic airway inflammation, and preclinical work in cell and animal models of asthma has shown reductions in key inflammatory markers such as eosinophil infiltration, mucus overproduction, and pro-inflammatory cytokines. On the clinical side, an open-label trial in asthma patients reported improvements in symptom frequency, lung function measures, and reduced medication use, and multiple trials in allergic rhinitis found butterbur performed comparably to standard antihistamines; however, at least one well-designed placebo-controlled trial found no significant benefit for intermittent allergic rhinitis, and another found butterbur ineffective at suppressing skin-based allergic responses, highlighting that results are not uniformly positive. The body of evidence includes systematic reviews, randomized controlled trials, and preclinical studies, with the strongest reviews noting that the most promising butterbur trials were often funded by the herb's manufacturer, that study sizes have generally been small, and that further independent, rigorously controlled research is needed before firm conclusions about efficacy can be drawn.
