Research suggests that bone marrow plays a central and dynamic role in immune function, serving as the primary site where blood and immune cells are generated, regulated, and reprogrammed in response to a wide range of internal and external signals, including infection, stress, inflammation, aging, and environmental exposures. The body of evidence reviewed here consists entirely of preclinical studies and review articles — primarily mouse models, mechanistic laboratory studies, and narrative reviews — with no randomized controlled trials or large-scale human clinical studies, which is an important limitation when considering how directly these findings translate to human health. Studies indicate that bone marrow's immune contributions extend well beyond simple cell production, with research demonstrating that it acts as a reservoir for immune cell mobilization during stress, a site of long-term epigenetic reprogramming following severe infection, a target of age-related deterioration that undermines immune output, and a participant in disease processes ranging from cancer immune evasion to cardiovascular inflammation. Findings are largely supportive of bone marrow's broad immunological importance, though several studies present mixed or complex results — for instance, evidence that the same bone marrow mechanisms that organize immune responses to stress can simultaneously impair the body's ability to fight certain infections, and that immune reprogramming originating in bone marrow stem cells can persist long after an initial trigger has resolved — underscoring that this tissue's role in immunity is not uniformly beneficial or straightforward.
