Research suggests that piperine, the primary bioactive compound in black pepper, demonstrates anti-inflammatory properties across a range of preclinical models, with multiple reviews and laboratory studies indicating it can reduce inflammatory markers, suppress certain immune cell activity, and modulate inflammatory signaling pathways. The evidence base includes numerous in vitro and animal studies, several narrative and systematic reviews, one randomized controlled trial comparing a piperine-containing herbal combination to naproxen in osteoarthritis patients with favorable results, and an early animal study showing partial but not complete suppression of inflammatory responses. Studies indicate that piperine may inhibit pro-inflammatory cytokines such as TNF-α, IL-1β, IL-6, and prostaglandin E2, and its immunomodulatory effects on immune cells like B lymphocytes have also been documented in laboratory settings. However, the overall body of evidence remains heavily weighted toward preclinical research, and multiple reviewers explicitly note that well-designed human clinical trials, cohort studies, and meta-analyses are still limited, meaning the translation of these findings to confirmed anti-inflammatory benefits in humans has not yet been fully established.
