Studies indicate that these effects are largely attributed to thymoquinone, the plant's primary active compound, which appears to act through anti-inflammatory and antioxidant pathways relevant to cardiovascular function. However, the evidence is not uniformly strong — a 2024 systematic review of RCTs in patients with metabolic syndrome found that only one of five trials showed significant blood pressure improvement, pointing to variability across study populations and conditions. Overall, while the direction of existing research is broadly supportive, reviewers consistently note that gaps between laboratory findings and clinical outcomes remain, and that larger, more rigorous human trials are needed before firm conclusions can be drawn.
