A 2026 systematic review of 158 studies on complementary menopausal therapies identified black cohosh among the better-supported options, though it characterized most of the available evidence as low or very low certainty. Additional support comes from two smaller pilot studies using multi-ingredient herbal formulas that included black cohosh alongside other botanicals, both of which reported sleep improvements, though their designs — one lacking a placebo control and one using a combination product — make it difficult to attribute the effects specifically to black cohosh. Overall, the direction of existing research is consistently supportive, but the evidence base remains limited in size and methodological rigor, and more large-scale, well-controlled trials are needed before firm conclusions can be drawn.
